For New Patients

Thank you for allowing us to be a part of your health care. We recognize that health care is one of the most important and personal decisions in your life. We appreciate your confidence in us. Our goal is to provide the best possible service to our patients. Please read this introductory information as it will help make your visit to us go as smoothly as possible.

YOUR FIRST VISIT

Please plan to arrive 10-15 minutes early for your appointment and bring with you the following:

We have three female physicians who share the on-call schedule. Therefore, if you are pregnant, you will meet all of them during your pregnancy. We also have two nurse practitioners who assist in providing care to you. All providers work very closely to coordinate your care. Meet our providers >>

PATIENT INFORMATION

You may download forms from our Web site to complete before you arrive for your first visit. Please complete all areas of the forms. If you have any questions about the history form, feel free to discuss it with your health care provider during your visit.

REFERRALS AND PRE-CERTIFICATION

If your visit requires a referral, please contact your primary care physician NOW to obtain the referral. If your visit requires pre-authorization by your insurance company, please contact us now so we can obtain this. We will not be able to see you if the necessary approvals are not obtained in advance of your visit.

INSURANCE CARDS

Please bring your insurance card(s) with you so we may make copies in order to file your services. If you do not have your insurance card(s), you will be required to pay in full for the services rendered at the time of your visit. You are also responsible for paying any co-payment or co-insurance amount at the time of your visit. Prior to your first visit, we may call your insurance company to obtain benefits information. We will notify you of your responsibility and set up a payment plan if necessary. Our office does not accept Medicaid or Hoosier Healthwise.

PRESCRIPTION REFILLS

Our staff provides this service at no fee if you call during our regular business hours; however, we are not able to refill or prescribe any medication until you are seen in the office by your provider.

TELEPHONE CONSULTATION AND QUESTIONS

We know that questions regarding your health can arise at any time. If you have non-emergency questions about your care, please call our office during normal business hours. You can direct your inquiries to your doctor's nurse or assistant, who will return all calls as soon as possible in the order they are received; however, if you think it is an emergency, please let the attendant know when you call. Routine calls received after 4:00pm may not be returned until the next business day.

CANCELLATION OF A SCHEDULED APPOINTMENT

We understand that situations arise that may prevent you from making a scheduled appointment. We request a 24-hour notice to cancel an appointment. This allows us to move a patient from our waiting list into your appointment time. If you repeatedly miss your appointments, a fee of $25 will be charged to your account. This fee will not be billed to your insurance company.

CONTACTING OUR OFFICE

Our phones are answered 24 hours a day. Outside of regular business hours (8:00am-5:00pm), our office number is answered by an answering service. It is possible that you will be put on hold before your call is answered. If this happens, please do not attempt to hang up and call back, as this will only delay your call. The doctor will be paged and she will answer your call within 15-30 minutes. If you feel you cannot wait for the doctor to call you back, go to the emergency room. If your call is not an emergency, please call us during normal business hours. Otherwise, you may incur a fee.

Here are some basic guidelines for when to call our office. These are examples only and are not all-inclusive. If you are unsure whether or not you truly have an emergency, please do not hesitate to call.

If you are pregnant...

Call immediately if you:

  • Experience heavy bleeding
  • Have severe abdominal pain
  • Begin labor contractions
  • Notice leaking fluid (not urine)
  • Notice decreased or absent fetal
    movements
  • Have a fever of 101.5 or higher
  • Are involved in an automobile collision
  • Experience allergic type reactions
  • Fall or injure your abdomen

Wait until normal office hours for:

  • Spotting
  • Vaginal discharge
  • Vaginal infection
  • Cold or flu symptoms
  • Trouble sleeping
  • Questions about medications
  • Prescription refills
  • To schedule or change an appointment
  • Obtain test results

 

Please check the list of medications (Adobe Acrobat Reader Required) you can and cannot take during your pregnancy. This should be a good guide for you throughout your pregnancy.

If you are not pregnant...

Call immediately if you:

  • Experience severe abdominal or pelvic pain

Wait until normal office hours for:

  • Spotting
  • Vaginal discharge
  • Vaginal infection
  • Cold or flu symptoms
  • Questions concerning medications
  • Prescription refills
  • To schedule or change an appointment
  • Obtain test results
  • Birth control questions
